Once this is received, the team will triage your referral and will contact you to arrange an appointment. If you require an urgent appointment, please ensure your referring doctor notes this on your referral.
We must receive the referral prior to making an appointment.
All patients are required to fill out a registration form and health questionnaire. The form can be found here.
To get the most from your appointment, please ensure all relevant results are sent to us prior to your consultation. This includes X-rays, CT, ultrasound or MRI scans, and blood tests.
